I received a medical form. Can you clarify the process for completing this form?
This form must be completed by your physician. The form is designed so that no additional attachments are required. This means that we should not receive copies of any test results, immunization records (except flu and COVID upon our request), etc.. The physician will simply list the dates of the results and indicate if any result was positive or negative. Your physician must sign the Medical Documentation form. Once you have completed the form, you can scan it and upload it on this website.
Why does Fox Chase Cancer Center require vaccinations?
Patient Safety and infection prevention is a priority at Fox Chase Cancer Center. Many cancer patients are immune-suppressed and this policy is in place for patient safety. All new applicants must provide the Medical Documentation Form providing proof of immunizations and health screenings before they can start (not necessary before Orientation). Your physician must sign the Medical Documentation form.
What is a 2-step tuberculosis (TB) test? Does that refer to the test being placed and read 48 hours later?
The TB skin test is used to determine if someone has an immune response to the bacteria that causes TB. The test is placed under the skin and is read 48 hours later. A second test is started 7-10 days later and read 48 hours later. This means that the 2-step TB test will require 4 visits with a medical provider. "2-step" does not refer to the steps of placing and reading the test, but the number of tests placed on the skin.
The medical form lists multiple tuberculosis (TB) tests. Can you explain which one I need?
Each applicant must complete at least one type of TB test. Please check with your insurance to see what is covered before obtaining a test, as many insurers will only cover a Tubeculin Skin Test (TST), sometimes also know as a PPD. The skin test must be a 2-step test. The test is placed under the skin and is read 48 hours later. A second test is started 7-10 days later and read 48 hours after it is placed. In lieu of a TB skin test, you may receive a blood test, such as a Quantiferon Gold (QFT) or TSPOT.
I received a TB test in the past six months, will you accept those results?
Yes, we accept the results of TB tests from the past 6 months or chest x-rays from the past year. If you received a TB skin test, you will be required to complete a second test, as we require a 2-step TB test. However, if you have completed a 2-step TB test (meaning you received a skin placement twice at least a week apart), we will accept those results for up to one year, but you will still be required to obtain one additional test.
I received a BCG vaccine for TB. What do I need to do?
If you received the BCG vaccine (administered in some countries but not in the USA), you may have a positive reaction to a TB skin test. Instead, you should submit a chest x-ray. The x-ray must have been completed in the past year or must be repeated if more than one year old.
Do I need to have a flu vaccine?
All Fox Chase Cancer Center employees and volunteers who are on-site between November 22nd through March 31st must obtain the flu vaccination or complete the declination form and wear a mask. Flu vaccinations are offered free of charge at FCCC for all volunteers at the beginning of each flu season. You may also receive the vaccine at another location and submit proof of vaccination on this website. Volunteers who are scheduled to start after March 31st do not need to submit proof of vaccination or a declination form for the current flu season.
Do I need a COVID vaccine?
All applicants must provide documentation of COVID vaccinations. The documentation should include date(s) and location(s) of the vaccine administration, vaccine name.
Do I need a Hepatitis B vaccine?
A Hepatitis B vaccination may be required for some volunteer assignments or volunteers may sign a declination form.
